HMO Patient Authorizations

Your request will be submitted to the Utilization Management Department of your Medical Group. The approval or denial documentation will be mailed to your address within 5-7 days. Please contact our office if you do not receive notice of your authorization. The contact information of the provider to whom you have been referred will be listed on the approved authorization letter. It is your responsibility as the patient to contact that provider and schedule an appointment.
